Medical Records Requests

Let us respond to these requests on your behalf. Here's what we'll need.

You may receive a request for medical records from your insurer either on an Explanation of Benefits notice, or in a separate letter. These requests are important as the information needed will likely be used to process reimbursements and payments.

Lakeside Billing Management will respond to these requests on your behalf. Please send us a copy of the request to records@lakesidebilling.com to be sure we have it and can follow up for you.

If you as a patient, your legal representative or guardian, or a designated third party are requesting records, a Release of Information must be received for us to release any record in accordance with Federal HIPPA Law.

You can obtain the Release of Information form from your healthcare provider or from Lakeside Billing. You may request the form by email at records@lakesidebilling.com, or via telephone (385) 225-3330 and ask for the "Medical Records Department."
